All providers can accept CareSource, Medical Mutual, United Healthcare, and UMR. All other insurances will be listed under each provider.

We are paneled with the following insurances, however not all therapists can accept every insurance. Please check with your therapist about which insurance they can accept.

• Ohio Medicaid

Each person is responsible for his/her co-payment and/or deductible. If you have a deductible to meet, you will pay the fixed rates below on the date service. If you are unaware of your co-pay and/or deductible, please call your insurance company.

If the copay or deductible is higher than the rates below, it is your responsibility to pay the remaining balance to Catalyst Counseling, LLC upon notification until your deductible is met.

Notify your therapist if there are any changes to your insurance benefits or if your insurance is discontinued.

*No copay or out of pocket costs for clients with Medicaid plans.
